Diabetes Care

Our goal is for you to live well with diabetes

Poorly controlled diabetes may cause a variety of complications including nerve damage, trouble healing wounds and problems with eyesight and circulation.

At the Diabetes Resource Center, we personalize your treatment plan and help to coordinate other services or specialists you may need to see.

We provide:

  • Referrals
  • Medications
  • Therapies

We will help you get control of the “ABCs” of diabetes:

  • A — Hemoglobin A1c
  • B — Blood pressure
  • C — Cholesterol

Your primary care provider can refer you to the Diabetes Resource Center. Our nurse practitioner will complete an individualized assessment and work together with you to set goals for your success.

You should use the Diabetes Resource Center if:

  • You have unanswered questions about your diabetes
  • You can’t seem to get your blood glucose under control
  • Your A1c is over 7.0 percent
  • You want to tailor your treatment to meet your goals
  • You are overwhelmed by the daily demands of living with diabetes

Prediabetes — Prevent T2

The National Diabetes Prevention Program, Prevent T2, is a proven lifestyle-change program offered in communities across the country. Adventist Health Sonora is pleased to offer this yearlong course aimed at reducing the number of diabetes diagnosis in our community by 50 percent by helping those with prediabetes reduce their risk of developing Type 2 diabetes. This program has been proven to help individuals improve food choices, increase physical activity, lose weight and succeed in making long-lasting changes leading to better health outcomes.

Prevent T2is open to anyone 18 years of age or older who meets the following criteria:

  • Available to attend most of the 25 meetings throughout the year
  • Body mass index of over 24 kg/m2
  • Recent blood test indicating prediabetes
  • Screens positive for prediabetes by taking this screening test

Screening Test

Physician referral is not required but is recommended. Enrollment is limited.

Diabetes Support Group

The Diabetes Support Group is for adults living with Type 1 and Type 2 diabetes. Each session includes a topic of interest and enough time for discussion. Light refreshments are also served. The support group is free, and no registration is required.
